    McCain's campaign is raising hell about Gwen Ifill, the moderator of tomorrow's VP debate. They say they just found out she's written a pro-Obama book, and that this is a conflict of interest which should disqualify her from serving as moderator.

    Just found out, my foot. I'll bet they knew all along but didn't say anything until they decided whether or not Sarah could hold her own in a debate. They're raising the book issue now only because it's clear Sarah is a disaster. The so-called conflict of interest is just a pretext to cancel or delay the debate.

    I say if they don't want Ifill as the moderator, then go ahead and replace her--with Helen Thomas.

    That was my hypothetical guess from last week. However, if it were Bell's, he wouldn't still be suffering with it now. It goes away after a few days.

    I've had Bell palsy before, it lasted 3 weeks, and is nothing like what his symptoms are. You can hardly blink at all with BP, and your lower eyelid won't close, it just sags, like the entire side of the face that is affected. Only treatment is maybe a little valium and sometimes prednisone and eyedrops to keep the eye from drying out, and a patch. It's caused from flu like illness with inner ear infection causing pressure on one of the cranial nerves temporarily. You don't twitch anything with Bell palsy.

    Bell's Palsy can last for months.

    What's clear is that something is wrong and no one knows what it is. It could be serious, or trivial.

    It's a disgrace that his medical records have not been released long enough for doctors to evaluate them. Bad enough for a regular candidate, but for someone 72 years old and with his health history, the MSM is totally falling down on their job to not make this a big issue. I guess they're afraid of being called ageists.

    I don't really see it, other than his usual twitchiness, which I've noticed. But that doesn't mean that there isn't something going on. His twitchiness could be an early sign of something that could be a more serious problem down the road. For his sake, I hope not. For the sake of the country, if it's going to happen -- I SAID, IF IT'S GOING TO HAPPEN -- I hope it happens before the election and not after.

    I took care of my father the last year of his life and was present when he had several mini-strokes. His face would convulse and he would become confused.....but then carry on in a normal way. However, he never remembered what had just happened.

    We just saw a mini-stroke.

    My grandma did the same thing. Mini-strokes... eye twitching, confused and slurring of words. She died months later after starting the mini-strokes. Very scary.

    If I had to guess I'd say he has something that I have: I don't remember the name, but it has something to do with interference to the nerves of the face due to stress. When it's acting up I'll get occasional "zaps" that feel like an electric shock to part of my face, usually across one eye. It starts just above an eyebrow and travels down through the eyelid into the cheek, and if it's nasty it can make me squint or clench that side of my face or make that eye twitch.

    My doctor says it's no big deal unless it happens violently and often. It comes and goes and seems to be related to sleep deprivation and stress. There's medication that can treat it.

    I've got it and I'm only 43; I picked it up at 42. At 72 McCain may have a bad case of it.

    When I listen to the news at the top of the hour.....or when I watch the nightly news.....the media plays a clip of what the candidate says that day......

    Well, the media always plays the best section/clip which represents what the candidate was talking about that day.

    Well, EVERYDAY, the "Clip of the day" is always McCain misprouncing a word.

    Pay close attention next time.....REALLY listen to what McCain is saying....and not just let the "sound clip" go in one ear and out the other ear.

    There are lots of things that can cause those twiches besides TIA's (strokes), many of them also serious neurological disorders. Seizures, clonic disturbances, meds, stress, sleep disturbances, combinations of those same things. Certainly repeated episodes warrant a doctor's examination and assurance to the public that nothing is wrong, if that is actually the case, which hasn't happened.

    I think you're over analyzing and hoping he's having a stroke or something... when he's just an old man being an old man.

    there are better things to slam him for that'll stand up to criticism a bit better.

    It seems that the blogosphere does influence the content of the MSM. Not evenly, but selectively. Consider how many times John and his peers guest on MSM talk shows. There must a reason. I believe bloggers influenced the MSM on the illegal occupation of Iraq, the elections, etc. It could happen on this issue, too. Never say never.

    That is why Americablog and other keep demanding that the medical records get released. Other than the 5-10 second per page hand picked journalists. If there is nothing to hide make the records public. If it is from the scar tissue let us see. As the Republicans were so fond of saying years ago - if you have nothing to hide you have nothing to fear.

    Just an observation from an Arizonan who has watched McCain for years. He's always been a little "ticcy" when he speaks. It would be helpful to try to get tape from various phases of his career and compare them to determine how significant this might be.

    I have also cared for elderly relatives who have suffered from these mini-strokes....seems like he's been having them lately... or more often since we're all noticing them.

    I find myself watching his left eye while he talks now and when he walks around, keep noticing that his left fist is typically clenched.

    Something is going on with the left side of his body....

    It looks like a tic, but it could be a seizure; that would explain its being followed by confusion (if that's what his apparent difficulty getting off stage represents.) Seizures would also explain why he sometimes seems like he is developing dementia and at other times seems perfectly clear and competent.

    Yes, but he wouldn't be expected to be speaking quite so fluently through a seizure, or a mini stroke. One would expect some type of verbal pause. Not always, but generally.

    John, it would be really helpful if you could show the entire clip from about 30 prior to the first instance of whatever you are seeing, because I'm just seeing his general squinty prune with a stick up his butt face.

    It's not Bell's Palsy, that's for sure.

    I hope everybody here is also doing something to follow up on this and annoy the hell out of CNN until they rerun it and rerun it and rerun it so that more people than just ourselves pick up on this medical event. One of the great skills of the right wing machine is its tenaciousness in haranguing the targets that need to be haranged, over and over, till something happens.

    Oh lord I hope he stays healthy. Obama is looking at an incredible and historic victory. It be a shame to have that taken away because his opponent bailed out (intentionally or not) because of health issues.

    I'm not insensitive to McCain's actual health problems - I definitely wish him best health at all time. It's just too bad he's hidden it all - now if something happens it'd come out of the blue for most people unaware of his problems.

    Another weird passing comment in the Wikipedia (hence not sure of it's validity)...

    Under the entry for Ptosis it lists people with it - Including Thom Yorke of Radiohead and actor Forrest Whittaker (I actually came across the entry linked from singer Gabrielle from England who also has it).

    It lists John McCain as a sufferer - though I couldnt find it in his own entry let alone this was added on the 25th of september per WIKI.

    I am not a physician, but I work in medical research. Part of my job is to abstract data from patient records. I can tell you that it is a very detail-oriented, time-consuming task, and that it helps to know what you are looking for and where in the record that information might exist. I think it would be very difficult for journalists, most of whom probably do not have a deep grounding in medical science and terminology, to cull out what is germane, especially with an extremely small window of time to do so. I am well-educated in the biological sciences and understand "medicalese", and still manage to learn something new every day.

    What we can take from his repeated confusion and tics is that it is legitimate to raise the question-"What is causing these and should it be of concern?".
    Certainly the facts at least raise the issue of his health and justify the release of his full medical records to determine his physical and mental fitness to lead.
    By not releasing the records he raises the question "What do you have to hide, Senator?".

    Just to be clear, if it is Bell's Palsy, there is nothing wrong with the left side of his face. It appears to be working. He blinks on that side and that side of his mouth went up. It's the right side that shows the effects of this type of paralysis. It also comes and goes and may be a stress issue as well.

    I have said this all along. McCain is to old to be running for President. This is just a power play with him. He cares nothing about america! I also believe he thinks he deserves to be President..what a joke. My god what is going to happen if he gets elected and Palin becomes President. This is crazy.

    I'm also sick about hearing about Biden going easy on her. This is an election and we need to win. I said hit her hard and keep hitting.

    I just recently lost my 46 year old sister on Aug 15 to Melanoma. She had 4 lesions removed since 2004.
    After the 3 lesion were removed the melanoma had sped into her lung and lymph nodes. The surgeon removed the lung tumor and affected lymph nodes. A few months later the scan reveled brain lesions. The Dr. Said that it was inoperable. That was last year Aug. 2007. She received experimental treatment and the brain growths were stable for 10 months. She always maintained her faculties until last April. She did develop a little twitch when she would become stressed. Other then that she never became confused nor did she have any seizures. So, I wouldn't be surprised if he had melanoma in his brain! Melanoma is the most aggressive cancer there is...and there is no known cure...

    My father suffered mini strokes, at least 9 before he had one that hospitalized him....it all began with a TIA (trans-ischemic-attack) while not a doctor or schooled in medicine, these are dangerous, and can lead to a severe stroke
    disabling or fatal. This guy's health is not up to the strain of campaigning or the duties of being president. Has anyone heard his voice? He sounds like he is slightly slurring his speech or is that his natural speech pattern? TIA's can cause temporary memory loss also.

    Well, I am a medical professional. I was an RN, an Advanced Nurse Practitioner and Certified Registered Nurse Anesthetist for 28 years, until an accident ended my career. However, John McCain's ptosis is not there consistently, or as dramatically, transient ischemic attacks could well be a cause of it. That is NOT a good thing. Ptosis can be caused by injury and can be permanent, or it can be transient due to MS, other neurological disorders or the cardiovascular effects of transient ischemic attacks. Anything of a transient nature can be due to mini strokes or a brain tumor or extreme stress. If it is stress, or any of these, he needs to get out of the kitchen.

    He is 72, and his blood pressure could be causing this, as blood vessels could be contracting and pushing up the pressure, and any person of that age has to have some atherosclerosis in their coronary arteries after eating American for that many years. The pressure goes up, and constriction in the already narrowed artery increases, and blood flow decreases and control of the face on that side, some confusion, and in some cases, even loss of conciousness ensues.

    It could also be from brain metastasis from his melanoma, as well.

    This is not the sort of thing one would want to see from a Presidential candidate. His health is very worrisome, and with his personality traits he will only sink further into the less attractive parts of his personality.

    Some time ago you had a dermatologist post here talking about melanoma, lets face it, McCain has had four bouts of the skin cancer and the most dangerous type. While he hasn't had a recurrance since what, 2000?, ther could be a metastasis happening elsewhere in his body. I do recall that Maureen Reagan, Pres. Reagan's eldest adopted daughter passed away from a brain tumour, which spread there from melanoma she had. Having had an experience eith my stepdaughter having a primary brain malignancy that affected her left eye and her peripheral vision, and seeing what I am seeing from John McCain...this is the first thing I thought of.

    Someone earlier said he'd had a significant tumor removed from the left side of his face; I believe that's what I remember reading elsewhere, also that there were lymph nodes removed and they were not affected. Perhaps injury from that combined with tiredness/stress could cause this; someone more medically knowledgeable could speculate better than me, but without knowing more details....

    Doesn't mean II think he should be running - for reasons of his health, or his policies, or his running mate.

    Transient ischemic attacks sounds (and looks) most likely, buuuut...only a complete diagnostic would determine. What are the chances McCain will suspend his campaign, see his doc, get referral, have an MRI/CatScan etc, anytime before the election? I'd say he is recklessly risking his health and life. If something more dramatic occurs it could change the whole dynamic of the race. Sarah Barracuda could soon be in the driver's seat, though I expect the Party wouldn't let that happen, not in a billion dollars, er, years.

    UPDATE: I see that questions about McCain's facial nerve problem have recently been (mostly) debunked by specialists familiar with McCain's history and knowledgeable about the condition he has: ptosis. What we see in the video appears not by itself a cause for concern.

    McCain has damage to his Seventh Cranial nerve, aka the facial nerve, as a consequence of his melanoma surgery and reconstruction. Here is some commentary by a surgical oncologist, and here is commentary by a neurologist.

    I thought it was a stroke, too (I'm an ER doc), but they make a compelling case.

    Both those articles are mainly concerned with ptosis of the left eye. After watching the clips, this was more than simple ptosis. It appears there is some type of involuntary contraction of the face followed by momentary confusion. Sure seems more like a TIA than neurological damage.

    I just saw the clip at normal speed and it makes a big difference. In slow motion it looks damning but in regular time it is a small tic or blip or whatever. Definitely not worth the all the arm chair guessing.

    it's facial synkinesis -- http://en.wikipedia.org/wiki/Synkinesis -- probably due to residual damage to the left facial nerve after his melanoma excision. the eye blinks shut when he tenses the left corner of his mouth.

    this is nothing like a stroke or a mini-stroke. it's not a tic, either. and it's not ptosis -- ptosis is a drooping of the eyelid, not a quick blink.

    While I usually do not watch McCain's speeches, I for some unknown reason watched it for a while. I kept thinking that he was blinking more than I had ever seen him do. That is the reason I quit watching. This is getting scary, considering he got lost leaving the stage.

    I agree with Shadowfox....I'm a nurse and have worked in large academic institutions in the midwest. I have 16 yrs experience in the Neurosciences.
    Mc Cain has experienced damage to the facial nerve. When nerves are damaged they tend to reinnervate(regrow) in an atypical pattern. So there can be a synkinesis with involuntary facial movements of the upper and lower face. I don't see that ptosis(drooping of an eyelid) is an issue here although there may be some blepherospasm(possible trigger) involved in the facial movement. These things in an of themselves are not necessarily indicative of TIA. Facial droop not synkinesis seems more likely expressed with TIA. It makes more sense that he had pretty radical surgery for melanoma on his face which has caused isolated nerve damage.

    The other night during the debate we decided to count how many times John McCain blinks. Over 100 times in one minute, that is pretty hard to do. We replayed the debate and tried to blink every time he did and it really is impossible. Try it next time you see him give a speech.

    So, maybe he has dry eyes, or maybe it is a way of releasing stress. Big deal.

    And maybe his facial tics are a sign of a serious underlying condition, or maybe not. No question he should have a neurological workup to determine that.

    I do agree that he should release his health records. The public has a right to know about his state of health, particularly since his chosen successor is a hockey mom from small town Alaska who can't name a single newspaper she reads regularly, thinks Iraq is a rack for eyes (not to mention a task from God), greets the President of Pakistan giggling and flirting as if she were some thirteen year old twit meeting everyone's favorite adolescent celebrity heartthrob, and believes being able to see some remote Russian island from some remote Alaskan island gives her insight into Russia (note that she has never claimed that she has actually gone to that remote Alaskan island and looked across the water at that remote Russian island - I guess just knowing it is there is sufficient for her purposes).

    And the fact that he is not being open about the health question does make one very suspicious.

    What we see in this clip is a "tic." Involuntary movement. Other symptoms of his left eye and face have been discussed elsewhere:

    The particular tic we see in this clip could be "tic doloreaux." This is also called "trigeminal neuralgia." If the previously-noted symptoms are indeed Horner's Syndrome, then there are few underlying causes for both trigeminal neuralgia and Horner's Syndrome. One cause could be post-surgical scarring and irritation of nerves from his last melanoma removal surgery. However, this surgery was years ago. If this were the cause, the symptoms should be stable over the past few years, not worsening or just developing now.

    The other major possibility is local recurrence of the melanoma he had in this area of his face. If so, symptoms would tend to worsen until he has treatment for the cancer recurrence.

    If John McCain were an applicant for any normal job, we'd insist on a careful, independent evaluation by a good neurologist. I fear that lower standards apply when the applicant is seeking the position of President of the United States.

    Actually, if John McCain were an applicant for any normal job, you would not be ALLOWED to ask any medical questions beyond asking whether he has any disability that might affect his ability to perform the duties of the job for which he was applying. If he said no, you would not be allowed to inquire further about his health. You would also not be allowed to know or ask about his age, nationality, race, sexual orientation, religion, etc., etc.

    It's the law.

    I am a retired internist but I taught neuroanatomy and did neurochemical research. Yes irritability of the seventh nerve pathways could be responsible for this phenonemon but there are other possibilities including somthing involving the facial nucleus or pontine pathways which could be a lesion but it seems unlikely that this would result in confusion. Confusion suggests a transient ischemic attack or a petti mal seizure. At the very least we need to have a neurological exam and an MRI revealed to the public before this man takes the office of president.

    Does McCain need a exam along with appropriate scans of the brain? ABSOLUTELY! Synkinesis is what you are seeing functionally when you watch the activity in his facial muscles. Could there be something more sinister going on.... like for example a recurrence of tumor or worse? YES. I have not watched the video but have looked at the stills closely. If in fact present, confusion and/or slurred speech could be related to TIA/stroke. If there was slurred speech it could also be related to the disorderly use of facial muscles due to the irregular nature of the nerve pattern.The fact that some so-called "specialists" write this off as ptosis is almost laughable. Anyone who knows medicine knows that that is not the case here. The man needs to have a full neurologic workup..... this could without a doubt be a sign of something more going on.

    McCain has the same facial tic I have, except mine's on the right. I'm pushing 60, and about 5 years ago, I survived the most horrifically stressful year of my life - during which time, I developed a facial tic. Mine does the same thing as his: my right eye closes and the corner of my mouth goes up. I'd bet anything McCain is stressed to the max and has also developed one.

    The wheels are coming off his campaign (due in large part to him), the CM (corporate media) is turning on him, Palin is becoming way too much fun for the Democrats (and SNL!), and what must be the most stressful part for McNasty is that he knows that if lets lose his temper, he's done for. That old man is stressed and pissed; he's going to explode well before election day! I can hardly wait.

    California is in trouble because the Gov and Rethugs refused to sign on to even a modest sales tax increase. It takes a 2/3s vote to pass the budget and Dems don't have that large a majority.
    Arnie promised to fix state finances. He's had six years to do that and the state is in worse financial shape now than when Grey Davis was recalled.
    There will probably be an initiative on the ballot next year to end the 2/3s requirement. Rethugs will scream like hell, but I think it may pass.

    2 things surprise of late. McCain's campaign made a big deal about pulling out of Michigan. He is taking the weekend off.

    Usually you just kind of slink out and see if anyone notices. You do not take the weekend off at the start of the last month of the election. The first is irrational, the second points to a health problem, exhaustion at the very least. Whatever it is 2 or 3 days is not going to heal it. My guess McCain is getting ready to withdraw "for health reasons". This opens a whole new can of election worms. Palin running, not likely. Postpone the election legally? Allow the repubs to chose another? It can be done. I hope not.

    If McCain dropped out, Palin moves to the top of the ticket. I think the Repubs could sramble with a ticket but the legistics of the election prohibit a complete re-write, at this point.

    The election happens: the ballots are printed and machines programmed with McCain's name. Alsa, voters would be asked to consider voting for a near-dead man who had otherwise dropped out.

    After reading through all the comments, there are certainly more qualified people than I am to address McCain's health.
    I would join with others in calling for his health records being made public. When a candidate is that old and has his health history, he should be required to show he is healthy enough to run this country for the next four years.

    I have epilepsy and the facial ticks that McCain is having remind me of what happens to me before I have seizures. My memory and concentration are impaired by my condition which is exaserbated by stress and sleep deprevation. Don't wake me up at 3 am and ask me to make a life and death decision. If McCain has been having petit-mal seizures (PMS LOL) no one would necessarily know because they basically just make you "zone out" for a few seconds as opposed to grand-mal seizures where you stop, drop, and flop. If McCain is having PMS (I love it) it would explain why he sometimes seems to lose focus in public, continues to make the same mistaken statements, exibits emotional swings, and forgets previously held beliefs. The bottom line is that the man is 72. He's too old for a lot of federal jobs and if indeed he has some form of epilepsy then he'd be disqualified from even more. He needs to lose this race and retire to AZ to BBQ with the kids and grandkids.

    I think the release of his medical records to be reviewed by doctors should be mandatory, so should any other candidate including the vice-presidents, the job is too important to think otherwise.

    There are a hundred legitimate reasons to criticize McCain, but this isn't one of them. Yes, he should disclose his medical records, but diagnosing him via TV is no more fair than right wingers diagnosing Terri Schiavo via TV without ever seeing her medical records.

    As you note, he's had several surgeries, including surgery to the left side of his face. It is entirely possible that he simply suffered some minor nerve damage as a result of the facial surgery. There are any number of possible explanations for this facial tick, both mundane and alarming.

    In my job, I frequently see people making presentations on stage. At least a couple times a month, a speaker gets momentarily befuddled figuring out which side to exit off of. A few times I've seen it happen to several people in one day. If you are concentrating on what you are giving a speech about or answering questions, it isn't all that uncommon to forget which way to go when you're done. It is a low priority, and sometimes underlings forget to tell them which way to go ahead of time, so it isn't even always their own fault.

    Yes, McCain should release his medical records. Yes, it is worthwhile to question the reason for these facial ticks. But it is ridiculous to leap to the assumption that there is something seriously wrong with him based on this alone. To make these assumptions makes you no better than Bill Frist.

    I'm late to this discussion, but the videos you and others provided offer a lot more information. I think the most likely cause is a disorder called, "hemifacial spasm", which is a diagnosis that is more descriptive than specific. It is caused by any number of problems affecting the seventh cranial nerve (the facial nerve) or its nucleus within the brainstem. Most times there is no identifiable cause. Therapy isn't easy or usually very successful without unacceptable side effects. It can be progressive - it may be that this is fairly early in its course, possibly enhanced by the stress of campaigning. The one important prediction of the diagnosis is that it is not a TIA, or otherwise predictive of a larger, more serious, neurologic problem.
